First Time Visit 1. Insurance information and Driver's License if you Referral from your primary care physician (if needed) Following your consultation and examination, the doctor
will determine if chiropractic treatment is appropriate for your condition.
If a positive determination is made, the doctor will outline treatment
options and possibly start treatment on the day of your first visit.
If chiropractic treatment is not appropriate the doctor will refer you
to a physician or specialist who can provide care that is appropriate
for your condition. 2. Medical information pertaining to the condition you are seeking help with including: a) Doctor's reports b) Diagnostic test results (X-rays, MRI, lab tests, etc.) c) List of medications currently being taken d) List of healthcare providers you are currently seeing or have seen for your condition
